简介:近年来,随着对Alzheimer病发病机制的不断深入研究,逐渐发现血管性因素在AD的发病及疾病进展方面起着重要作用,从流行病学、神经病理学、神经影像学、神经分子生物学等研究领域不断有证据证实这一点.传统观点认为AD是老年变性病,病因与遗传、病毒感染、炎症、铝中毒、胆碱系统功能缺陷、细胞骨架改变以及颅脑外伤有关,均未提及与血管性因素有关,而是把AD和血管性痴呆(VascularDementia,VaD)截然分成变性和血管病两个概念,把是否有脑血管病作为区分AD与VaD的要点,显然这样的分类已显示出局限性.血管性认知障碍(vascularcognitiveimpairment,VCI)这一概念的提出为AD的归属提供了新的方向,AD是否应该属于VCI的范畴呢?这个问题的澄清不单是简单意义上概念的转换,而将会为AD的诊断和早期防治提供一条新思路.
