  • 简介:AbstractDue to the increase in human population, habitat destruction, and the close interaction between humans, animals and the environment, these dynamic changes are a threat to the health of human, animal and environment. Therefore, we required intervention consisting of medical, veterinary, environment and other relevant disciplines and sectors in combination. If biological and environmental health is supposed to be optimum, cooperation and coordination is necessary between the multi-sectoral stakeholders responsible for the efforts. It not limits to a region or country on a worldwide scope. One Health encourages a collaborative, coordinated, and multidisciplinary approach to ensure the health and wellbeing of humans, animals, and the environment across different spatial levels, it aims to optimal health for lives and the environment, and recognizing the interconnection between people, animals, plants, and their shared environment. This review discussed the essential to integrate One Health in the areas of infectious diseases, food safety, antimicrobial resistance, ecological environment, and chronic diseases etc., and discuss the practice of One Health in each area with some examples, hoping that One Health will serve as a framework to solve the challenges and issues facing China.

  • 简介:AbstractBackground:A One Health approach has been increasingly mainstreamed by the international community, as it provides for holistic thinking in recognizing the close links and inter-dependence of the health of humans, animals and the environment. However, the dearth of real-world evidence has hampered application of a One Health approach in shaping policies and practice. This study proposes the development of a potential evaluation tool for One Health performance, in order to contribute to the scientific measurement of One Health approach and the identification of gaps where One Health capacity building is most urgently needed.Methods:We describe five steps towards a global One Health index (GOHI), including (i) framework formulation; (ii) indicator selection; (iii) database building; (iv) weight determination; and (v) GOHI scores calculation. A cell-like framework for GOHI is proposed, which comprises an external drivers index (EDI), an intrinsic drivers index (IDI) and a core drivers index (CDI). We construct the indicator scheme for GOHI based on this framework after multiple rounds of panel discussions with our expert advisory committee. A fuzzy analytical hierarchy process is adopted to determine the weights for each of the indicators.Results:The weighted indicator scheme of GOHI comprises three first-level indicators, 13 second-level indicators, and 57 third-level indicators. According to the pilot analysis based on the data from more than 200 countries/territories the GOHI scores overall are far from ideal (the highest score of 65.0 out of a maximum score of 100), and we found considerable variations among different countries/territories (31.8-65.0). The results from the pilot analysis are consistent with the results from a literature review, which suggests that a GOHI as a potential tool for the assessment of One Health performance might be feasible.Conclusions:GOHI—subject to rigorous validation—would represent the world’s first evaluation tool that constructs the conceptual framework from a holistic perspective of One Health. Future application of GOHI might promote a common understanding of a strong One Health approach and provide reference for promoting effective measures to strengthen One Health capacity building. With further adaptations under various scenarios, GOHI, along with its technical protocols and databases, will be updated regularly to address current technical limitations, and capture new knowledge.

  • 简介:AbstractBackground:Zoonoses are public health threats that cause severe damage worldwide. Zoonoses constitute a key indicator of One Health (OH) and the OH approach is being applied for zoonosis control programmes of zoonotic diseases. In a very recent study, we developed an evaluation system for OH performance through the global OH index (GOHI). This study applied the GOHI to evaluate OH performance for zoonoses in sub-Saharan Africa.Methods:The framework for the OH index on zoonoses (OHIZ) was constructed including five indicators, 15 subindicators and 28 datasets. Publicly available data were referenced to generate the OHIZ database which included both qualitative and quantitative indicators for all sub-Sahara African countries (n = 48). The GOHI algorithm was used to estimate scores for OHIZ. Indicator weights were calculated by adopting the fuzzy analytical hierarchy process.Results:Overall, five indicators associated with weights were generated as follows: source of infection (23.70%), route of transmission (25.31%), targeted population (19.09%), capacity building (16.77%), and outcomes/case studies (15.13%). Following the indicators, a total of 37 sub-Sahara African countries aligned with OHIZ validation, while 11 territories were excluded for unfit or missing data. The OHIZ average score of sub-Saharan Africa was estimated at 53.67/100. The highest score was 71.99 from South Africa, while the lowest score was 40.51 from Benin. It is also worth mentioning that Sub-Sahara African countries had high performance in many subindicators associated with zoonoses, e.g., surveillance and response, vector and reservoir interventions, and natural protected areas, which suggests that this region had a certain capacity in control and prevention or responses to zoonotic events.Conclusions:This study reveals that it is possible to perform OH evaluation for zoonoses in sub-Saharan Africa by OHIZ. Findings from this study provide preliminary research information in advancing knowledge of the evidenced risks to strengthen strategies for effective control of zoonoses and to support the prevention of zoonotic events.

  • 简介:AbstractSince its outbreak, the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has strongly influenced the life of the general public around the world. Based on its fast spread and high mortality, there is a need for novel therapeutic treatments to overcome this global health crisis. While medicinal chemistry is focused on the development of highly selective and affine inhibitors toward a specific target enzyme, material science is focused on the development of nanomaterials for selective drug delivery. Based on the individual strengths, these disciplines could synergistically act together and help overcome the limitations of the respective approach. Herein, the combination of medicinal chemistry with material science to overcome health problems with the example of SARS-CoV-2 is critically discussed.

  • 简介:AbstractAntimicrobial peptides (AMPs) are the natural antibiotics recognized for their broad-spectrum resistance to bacteria, fungi, viruses and parasites, and influencing the host immune responses. AMPs attributed to good biological effects have been used in various areas of human health, which are trying to completely replace antibiotics, owing to serious drug-resistance bacteria. However, limited bioactivity and potential biotoxicity of some AMPs was neglected, attributable to their hydrophobic structure with positive charges and nonspecific destruction of cell membranes. Various strategies have been used to design and biosynthetic optimized AMPs to improve their bioactivity, productivity, while lowering host toxicity and cost. Here, we focus on the progress made in understanding the AMPs, including biosynthesis (AMP-BioDesign 1.0 and 2.0), bioactivity (e.g. immune regulation and broad-spectrum or nonspecific actions against bacteria, viruses or parasites), and principle biotoxicity (e.g. hemolysis, acute toxicity and instability, ect). The application prospects of AMP for human health, clinical medicine (for novel drugs), tissue engineering and drug delivery system, respectively, are summarized in this review. Furthermore, future prospects and new strategy for the development of effective and low-toxic AMP formulations for human health are discussed.

  • 简介:AbstractRecently, death from herpes B virus (Cercopithecine Herpesvirus 1) infection was reported in China, reminding us to be alert to the risk of transmission and infection with herpes B virus. Herpes B virus is a zoonotic pathogen that can cause fatal encephalomyelitis in humans. The virus naturally infects rhesus monkeys, causing diseases like human herpes simplex virus (HSV) infection, which are often asymptomatic or mild in the rhesus macaque. However, herpes B virus infection can be fatal to humans. Without timely treatment, the mortality rate of herpes B virus infection is as high as 70%-80%. To date, approximately 50 cases of human herpes B virus (HBV) infection have been reported worldwide. Most cases are related to direct contact with macaques, such as scratching, biting, or mucosal contact with monkey body fluids or secretions. Although the risk of human-to-human transmission is low, the widespread prevalence of the virus among monkeys, the high mortality of infected persons, and the severe neurological sequelae of survivors render this virus an important zoonotic pathogen that threatens human beings. Biorisk-related training programs for at risk personnel and timely treatment after exposure to the B virus can reduce infection rate and mortality. The early initiation of antiviral therapy prevents severe disease or death after the rapid diagnosis of human B virus disease. Identification of risk factors is essential in controlling the spread of the herpes B virus in the population at risk.

  • 简介:AbstractBackground:The One Health (OH) concept has been promoted widely around the globe. OH framework is expected to be applied as an integrated approach to support addressing zoonotic diseases as a significant global health issue and to improve the efficiency and effectiveness of zoonosis prevention and control. This review is intended to overview the social impact of the implementation of OH on zoonosis prevention and control.Methods:A scoping review of studies in the past 10 years was performed to overview the integration feature of OH in zoonosis prevention and control and the social impacts of OH. PubMed and Web of Science were searched for studies published in English between January 2011 and June 2021. The included studies were selected based on predefined criteria.Results:Thirty-two studies were included in this review, and most of them adopted qualitative and semi-qualitative methods. More than 50% of the studies focused on zoonosis prevention and control. Most studies were conducted in low- and middle-income countries in Africa and Asia. Applying OH approach in diseases control integrates policymakers, stakeholders, and academics from various backgrounds. The impact of OH on economic is estimated that it may alleviate the burden of diseases and poverty in the long term, even though more financial support might be needed at the initial stage of OH implementation. OH implementation considers social and ecological factors related to zoonosis transmission and provides comprehensive strategies to assess and address related risks in different communities according to regions and customs.Conclusions:Based on reviewed literature, although there seems to be a lack of guidelines for assessing and visualizing the outcomes of OH implementation, which may limit the large-scale adoption of it, evidence on the contributions of implementing OH concepts on zoonosis prevention and control indicates long-term benefits to society, including a better integration of politics, stakeholders and academics to improve their cooperation, a potential to address economic issues caused by zoonosis, and a comprehensive consideration on social determinants of health during zoonosis prevention and control.

  • 简介:AbstractViral infections during pregnancy are associated with adverse pregnancy outcomes, including maternal and fetal mortality, pregnancy loss, premature labor, and congenital anomalies. Mammalian gestation encounters an immunological paradox wherein the placenta balances the tolerance of an allogeneic fetus with protection against pathogens. Viruses cannot easily transmit from mother to fetus due to physical and immunological barriers at the maternal-fetal interface posing a restricted threat to the fetus and newborns. Despite this, the unknown strategies utilized by certain viruses could weaken the placental barrier to trigger severe maternal and fetal health issues especially through vertical transmission, which was not fully understood until now. In this review, we summarize diverse aspects of the major viral infections relevant to pregnancy, including the characteristics of pathogenesis, related maternal-fetal complications, and the underlying molecular and cellular mechanisms of vertical transmission. We highlight the fundamental signatures of complex placental defense mechanisms, which will prepare us to fight the next emerging and re-emerging infectious disease in the pregnancy population.

  • 简介:AbstractSince its first discovery, the Omicron variant of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has evoked another wave of infection and caused global concern and panic. Moreover, although the data are still limited, Omicron showed highly concerning characteristics, including higher transmissibility, extensive immune escape and potentially altered host range. We interpreted these characteristics based on currently available data and outlined some urgent questions, calling for a more comprehensive investigation.

Adapting climate change should not only be a slogan, but requires a united, holistic action and a paradigm shift from crisis response to an ambitious and integrated approach immediately. Recognizing the urgent needs to tackle the risk connection between climate change and One Health, the four key messages and recommendations that with the intent to guide further research and to promote international cooperation to achieve a more climate-resilient world are provided.

  • 简介:AbstractChina has accumulated multiple practices and experiences in building and enhancing malaria surveillance and response system. As China’s engagement into global health has gathered stronger momentum than ever, China together with the Swiss Tropical and Public Health Institute and WHO has organised five sessions of the International Forum on Surveillance-Response System Leading to Tropical Diseases Elimination during 2012-2020, in which malaria elimination has always been one of the hottest topics. In this study, the roles of international network on the surveillance and response system were explored to achieve a global malaria-free goal. China’s approach to malaria elimination has demonstrated significance of global collaboration on taking joint prevention and control, and building a worldwide institutional-based network.

  • 简介:AbstractBackground:In-hospital mortality in patients with coronavirus disease 2019 (COVID-19) is high. Simple prognostic indices are needed to identify patients at high-risk of COVID-19 health outcomes. We aimed to determine the usefulness of the CONtrolling NUTritional status (CONUT) index as a potential prognostic indicator of mortality in COVID-19 patients upon hospital admission.Methods:Our study design is of a retrospective observational study in a large cohort of COVID-19 patients. In addition to descriptive statistics, a Kaplan-Meier mortality analysis and a Cox regression were performed, as well as receiver operating curve (ROC).Results:From February 5, 2020 to January 21, 2021, there was a total of 2969 admissions for COVID-19 at our hospital, corresponding to 2844 patients. Overall, baseline (within 4 days of admission) CONUT index could be scored for 1627 (57.2%) patients. Patients’ age was 67.3 ± 16.5 years and 44.9% were women. The CONUT severity distribution was: 194 (11.9%) normal (0-1); 769 (47.2%) light (2-4); 585 (35.9%) moderate (5-8); and 79 (4.9%) severe (9-12). Mortality of 30 days after admission was 3.1% in patients with normal risk CONUT, 9.0% light, 22.7% moderate, and 40.5% in those with severe CONUT (P < 0.05). An increased risk of death associated with a greater baseline CONUT stage was sustained in a multivariable Cox regression model (P < 0.05). An increasing baseline CONUT stage was associated with a longer duration of admission, a greater requirement for the use of non-invasive and invasive mechanical ventilation, and other clinical outcomes (all P < 0.05). The ROC of CONUT for mortality had an area under the curve (AUC) and 95% confidence interval of 0.711 (0.676-0746).Conclusion:The CONUT index upon admission is potentially a reliable and independent prognostic indicator of mortality and length of hospitalization in COVID-19 patients.

  • 简介:AbstractBackground:Lymphatic filariasis (LF) remains one of the world’s most debilitating parasitic infections and is a major contributor to poor health in many endemic countries. The provision of continuing care for all those affected by LF and its consequences is an important component of the United Nations’ Sustainable Development Goals. The aim of this study is to integrate lymphedema care into the primary health care system of the State by developing lymphedema clinics at each district, through training of health personnel to fulfill WHO recommendation for morbidity management and disability prevention.Methods:Selected health care providers from all the districts in Kerala State of India participated in intensive training sessions endorsed by the State’s health administration. The six training sessions (from 5 June 2017 to 25 May 2018) included appropriate self-care information and development of individual plans for each participating institution to provide instruction and care for their lymphoedema patients. The learning achieved by attendees was assessed by pre- and post-training tests. The number of lymphoedema patients receiving care and instruction from the post-training activities of each participating institution was assessed from local records, 6 months after the conclusion of the training sessions.Results:One hundred and eighty-four medical personnel (91 doctors and 93 nurses) from 82 medical institutions were trained which quickly led to the establishment of active lymphoedema clinics providing the essential package of care (EPC) for lymphoedema patients at all the participating institutions. Six months after the training sessions the number of previously unidentified lymphoedema patients registered and receiving care at these clinics ranged from 296 to almost 400 per clinic, with a total of 3,477 new patients receiving training in EPC.Conclusions:Generalist health personnel, when appropriately trained, can provide quality lymphoedema care in public health settings and patients when provided services close to their home, are willing to access them. This is a feasible strategy for integrating long term care for LF patients into the national health system, and is a clear example of moving towards equity in health care for the medically underserved, and thus successfully addresses a major goal of the global program to eliminate lymphatic filariasis.
