血清降钙素原联合脑脊液常规检测在颅内感染鉴别诊断中的临床应用探讨

摘要    [摘要 ] 目的 探究血清降钙素原联合脑脊液常规检测在颅内感染鉴别诊断应用的临床价值。方法 整群选择 2017年 12月— 2019年 12月该院行颅脑手术的 154例患者为研究对象,依照患者术后是否发生感染, 将其分成对照组 89例(未感染)和实验组 65例(已感染),对患者的相关情况进行观察,统计出阳性和阴性预测值,特异性,灵敏性。结果 和对照组相比,实验组患者的血清,脑脊液 PCT和体温明显较高, P<0.05.葡糖水平显著偏低, P<0.05.WBC计数,中性粒细胞百分比有所上升,但与对照组相比差异不显著。脑脊液 PCT对颅内感染诊断特异性为 62.3%。敏感性为 97.1%。治疗前,两组患者的蛋白定量,白细胞计数和血清 PCT指标存在差异有统计学意义,治疗后,蛋白质和白细胞计数差异无统计学意义, PCT血清水平存在差异有统计学意义;病毒组的治疗前后指标差异无统计学意义,细菌组差异有统计学意义。结论 使用血清 PCT联合脑脊液 PCT及常规检验联合检测,有着较高的敏感度和特异度,对于预测患者术后颅内感染情况具有非常重要的现实意义,值得进一步推广。     [关键词 ] 颅内感染;降钙素原;脑脊液;血清    [Abstract] Objective To explore the clinical value of serum procalcitonin combined with routine cerebrospinal fluid detection in the differential diagnosis of intracranial infection. Methods a total of 154 patients who underwent craniocerebral surgery in our hospital from December 2017 to December 2019 were selected as the study objects. According to whether the patients had postoperative infection or not, they were divided into control group (89 cases) and experimental group (65 cases) respectively. The relevant conditions of the patients were observed and the positive and negative predictive values, specificity and sensitivity were calculated. Results compared with the control group, the PCT and body temperature in the serum, CSF of the experimental group were significantly higher, P < 0.05. The glucose level was significantly lower, P < 0.05. WBC count, the percentage of neutrophils increased, but there was no significant difference compared with the control group. The specificity of CSF PCT in the diagnosis of intracranial infection was 62.3%. The sensitivity was 97.1%. Before treatment, there were significant differences in protein quantity, white blood cell count and serum PCT index between the two groups. After treatment, there was no significant difference in protein and white blood cell count, and there was significant difference in PCT serum level. There was no significant difference in virus group before and after treatment, and there was significant difference in bacteria group. Conclusion the combination of serum PCT with CSF PCT and routine test has high sensitivity and specificity, which is of great practical significance for the prediction of postoperative intracranial infection, and is worthy of further promotion.
