3.0T磁共振扩散加权成像在乳腺良恶性病变鉴别诊断中的应用

摘要   【摘要】 目的:分析 3.0T磁共振扩散加权成像( DWI)在乳腺良恶性病变鉴别诊断中的应用价值。方法:选取 2018年 1月 1日 -2019年 1月 1日笔者所在医院收诊的乳腺癌患者 35例作为恶性病变组,乳腺良性病变患者 30例作为良性病变组,正常乳腺者 30例作为正常组,均进行 3.0T DWI检查,对比三组的检查结果。结果:三组的不同 b值的平均 ADC值比较,差异均有统计学意义( P<0.05)。三组同一 b值的平均 ADC值比较,差异均有统计学意义( P<0.05)。恶性病变组 ADC值≤ 0.97 mm2/s 29例, >0.9 7mm2/s 6例,良性病变组 ADC值≤ 0.97 mm2/s 3例, >0.97 mm2/s 27例,恶性病变组 ADC值整体低于良性病变组,差异有统计学意义( P<0.05)。 b值为 1200 s/mm2时,敏感性、特异性、准确性更高,差异有统计学意义( P<0.05), b值为 1200 s/mm2时更具有应用价值。结论: 3.0T磁共振扩散加权成像在乳腺良恶性病变鉴别中具有较高的准确性、特异度与敏感性,是一种可行性较高的鉴别诊断方法。    【关键词】 3.0T磁共振扩散加权成像; 乳腺良恶性病变; 鉴别诊断; 价值    [Abstract] Objective: to analyze the value of 3.0T diffusion weighted imaging (DWI) in the differential diagnosis of breast benign and malignant lesions. Methods: from January 1, 2018 to January 1, 2019, 35 patients with breast cancer, 30 patients with benign breast disease and 30 patients with normal breast disease were selected as the malignant disease group, 30 patients with benign breast disease as the benign disease group and 30 patients with normal breast as the normal group, all of them were examined with 3.0T DWI, and the results of the three groups were compared. Results: there was significant difference in ADC between the three groups (P < 0.05). There was significant difference in the average ADC value of the same b value among the three groups (P < 0.05). There were 29 cases with ADC value ≤ 0.97 mm2 / s, 6 cases with ADC value > 0.97 mm2 / s, 3 cases with ADC value ≤ 0.97 mm2 / s and 27 cases with ADC value > 0.97 mm2 / s in the malignant group, and the ADC value in the malignant group was lower than that in the benign group, the difference was statistically significant (P < 0.05). When the b value is 1200 s / mm2, the sensitivity, specificity and accuracy are higher, and the difference is statistically significant (P < 0.05). When the b value is 1200 s / mm2, it is more applicable. Conclusion: 3.0T MR diffusion weighted imaging has high accuracy, specificity and sensitivity in the differential diagnosis of breast benign and malignant lesions, which is a feasible differential diagnosis method.
